Cut 2 pieces of 12x12 paper into 1 inch strips. Then weave together over and under alternating each strip. Mat the photo on coordinating cardstock. Add embelishments and tag with ribbon. Use stickers for title on tag. Don't forget to use your very own imperfect handwriting for your child to treasure!
